Accreditation Information

Verification of Status as Accredited Investor.  

Instructions: The Company is required to independently verify that an Investor is an accredited investor by one of the following methods. Please check the box below if you agree to provide us with one of the verification methods of your status.

• A verification letter from one of the following persons: a broker dealer or investment adviser registered with the United States Securities and Exchange Commission; a licensed attorney who is in good standing under the laws of the jurisdictions in which he or she is admitted to practice law; or a certified public accountant who is duly registered and in good standing under the laws of the place of his or her residence or principal office.
  • The verification letter confirms that the verifier has taken reasonable steps to verify that the Investor is an accredited investor within the prior three months and has determined that such Investor is an accredited investor. 
• A verification of the Investor’s income for the prior two calendar years, which verification may include income tax returns filed with the United States Internal Revenue Service or comparable foreign governmental agency or other evidence reflecting such income.
• A balance sheet dated as of a date within the past three months, listing assets, liabilities, and net worth and verification of the value of such assets, such as bank statements, brokerage statements, tax assessments and appraisals, and independent confirmation of the amount of such liabilities, such as a credit report.
• Evidence that Investor holds one of the following licenses in good standing: the General Securities Representative license (Series 7), the Private Securities Offerings Representative license (Series 82), or the Investment Adviser Representative license (Series 65).
